What to bring

Personal
  • Full legal name, date of birth and social security number for yourself, your spouse and any others you are claiming as a dependent
  • Prior year tax return (if a new client)
Employment/Income​
  • W2 for each job for each person from above
  • 1099-INT, 1099-DIV, 1099-OID, 1099-B for investment or interest income statements
  • K-1 and 1099-MISC for all self employment income
  • 1099-R for pensions, IRAs and annuities
  • 1099-SSA and 1099-RRB for social security/RRB income
  • Rental property income and expenses
  • Support for other income such as unemployment/state tax refund (1099-G), gambling income (W-2G), alimony received, health savings account (1099-SA), long-term care reimbursements (1099-LTC), jury duty pay, scholarships and fellowships
Health Care Coverage
  •  Form 1095-A if enrolled in insurance plan through the Exchange
  • Form 1095-B and/or 1095-C if you had coverage through employer, insurance coverage or government health plan
Deductions
  • Form 1098 for mortgage interest
  • Real estate taxes paid and personal property tax records
  • Closing statement(s) for sale/purchase of primary residence
  • Form 1098-T and itemized receipts for qualified educational expenses
  • Form 1098-E for student loan interest
  • Form 5498 for IRA contributions
  • Form 5498-SA for Health Savings Account contributions
  • Invoice showing amount of vehicle sales tax paid
  • Donor letter for cash amounts donated to charitable organizations
  • Records for non-cash contributions
  • Records for medical and dental costs
  • Records of estimated tax payments paid
  • Alimony expenses paid
Additional Information
  • Legal papers for adoption, divorce, or separation involving custody of your dependent children​
  • Notice 1444 from the IRS showing the amount of the Economic Impact Payment (EIP) you received
  • Letter 6419 from the IRS showing the amount of advance Child Tax Credit payments you received
  • Any tax notices sent to you by the IRS or other taxing authority
  • Six-digit Identity Protection PIN if sent to you by the IRS
